How To Submit A Form

  1. Download the MCPS Canvas App
  2. After you complete the form and have required signatures open the Canvas App
  3. Navigate to Assignments and choose the month you completed the service and choose either (SENIORS ONLY) or (Grades 9-11 ONLY) based on your grade level.
  4. Follow the on screen directions for scanning and upload your form.
  5. Keep the form and look out for a response via Canvas about your submission in the next few days to weeks.
