Borderline viability
You are the RN in Labor and Delivery at a tertiary care hospital with a Level IV NICU. It is 0130. A 26 y/o G2P0 (previous loss at 19 weeks) patient at 22 weeks and 4 days gestation presents via ambulance with advanced cervical dilation, bulging membranes, and regular contractions. She has a known diagnosis of cervical insufficiency diagnosed at 20 weeks but declined the cerclage. There is only a 10% to 30% survival rate for a baby this age and complications are probable including severe intraventricular hemorrhage, chronic lung disease, cerebral palsy, blindness, and neurodevelopmental impairment. Aggressive resuscitation is contraversal at 22 weeks. Neonatology explains that survival at this gestational age is very low and severe disability is likely. The mother wants the health care team to do everything possible to resuscitate the baby. The partner is overwhelmed and unable to respond.
This patient presents to labor and delivery with 6 cm dilation, bulging membranes, Fetal Heart Rate 148 beats per minute, No signs of infection, contractions every 4 minutes
