Pinch the digit and observe flexion of the entire limb.Note: a complete/normal reflex should demonstrate complete flexion of shoulder, elbow, carpus. Note: an intact withdrawal alone does NOT confirm pain perception.
While testing the withdrawal reflex in lateral recumbency, observe the opposite limb for inappropriate extension, which may indicate loss of normal upper motor neuron inhibition.
