There is a laceration or a hematoma.Solution: repairing the laceration or evacuation of hematoma.
The patient has some sort of coagulopathy.Solution: Identify, replace lost products/clotting factors
There is retained tissue. Resolution- Either a D&C or medication, depending upon full clinical picture.
The uterus is boggy. Uterotonic medications should do the trick!

The ED physician has consulted the OB/GYN on call and the patient will require a dilation and curettage(D&C) procedure

The patient should not be left alone
Although the thought to have her clean up is kind and empathetic, this patient should not be left alone, especially behind closed doors of a bathroom. There is risk of hypotension and can lead to a fall. If she is requesting to clean up, someone should assist her. Consider it an opportunity to begin a visual assessment.
This patient is a priority
This patient is priority due to hemodynamic instability related to blood loss. She should be seen/assessed by the nurse immediately.

Perinatal patients are different!
One thing that separates pregnant and postpartum people from the general population is that they will loose about 20-25% of their blood volume before they show any change in vital signs. This is due to the hemodynamic and cardiovascular changes that happen during pregnancy. Their body is already preparing for blood loss. Meaning- vital sign changes are a LATE sign of distress in an pregnant or postpartum person.
