HISTORY OF PRESENT ILLNESS

Oliver, 8 year old Caucasian male, presents with mother and father at 09:20 pm for dental trauma. Per mother, Patient was playing outside in the driveway, when he placed his hat down to his face without being able to see were he was going and struck his face with the front of a truck. Incident happened around 04:30 pm. Witness of event: Neighbor and Father. No loss of consciousness or vomiting reported.

Dwayne, 12 year African American male, presents with father at 04:53 pm for dental trauma. Per father and patient, Dwayne was playing basketball at school when he accidentally tripped and fell, hitting his mouth/teeth . The incident happened around 2:00 pm. Witness of event: Team players. No loss of consciousness or vomiting reported.

Chance, 9 year Caucasian male, presents with mother at 7:50 pm for dental pain. Per mother, He came from school and told her he had been in pain all day. Per patient, he tried to pull the tooth but felt dizzy and couldn't.
