COMPTE-RENDU D'EXAMEN : ÉCHOCARDIOGRAPHIE TRANSTHORACIQUE (ETT) PATIENT : Mademoiselle BERNIER Chloé DATE : 20/01/2026 MÉDECIN : Dr. MOREAU, Cardiologue INDICATION : Bilan de malaises à l’effort et suspicion de cyanose d'effort chez une patiente de 17 ans.

DESCRIPTION : • Situs et connexions : Cœur en place, connexions auriculo-ventriculaires et ventriculo-artérielles normales. • Cavités gauches : Le ventricule gauche présente un diamètre diastolique normal (46 mm). Pas d'hypertrophie des parois. La fraction d'éjection (FEVG) est excellente, évaluée à 68 %. L'oreillette gauche n'est pas dilatée. • Cavités droites : Le ventricule droit est de taille normale, avec une bonne contractilité. L'oreillette droite présente une morphologie normale au repos. • Appareil valvulaire : Valves mitrale, aortique et tricuspide fines et mobiles. Absence de fuite ou de sténose significative. • Cloison inter-auriculaire (CIA) : La cloison semble intacte en mode B (noir et blanc). Aucun flux de passage n'est visualisé au Doppler couleur dans les conditions de repos. • Péricarde : Absence d'épanchement péricardique. CONCLUSION : Examen strictement normal au repos. Aucune anomalie structurelle ou fonctionnelle n'explique les symptômes rapportés. La continuité de la cloison inter-auriculaire paraît respectée. Note : Compte tenu de la symptomatologie clinique (cyanose à l'effort), une épreuve de contraste (test aux microbulles) est préconisée pour rechercher un Foramen Ovale Perméable (FOP) non visible au repos.

Résultats : Au repos : Les microbulles remplissent massivement l'oreillette droite (OD). Aucune bulle n'est observée dans l'oreillette gauche (OG) sur 10 cycles cardiaques. À l'effort (Manœuvre de Valsalva) : Suite à la levée de la pression (on demande à Chloé de bloquer sa respiration (effort), ce qui augmente la pression à droite), on observe un passage massif de microbulles (> 20 bulles) de l'OD vers l'OG en moins de 3 cycles.

Examen : Échocardiographie transthoracique (ETT). Observations : • Cavités cardiaques : Les dimensions des ventricules et des oreillettes sont dans les normes. Pas de dilatation constatée. • Myocarde : L'épaisseur du muscle cardiaque est normale. La contractilité est conservée (Fraction d'Éjection du Ventricule Gauche - FEGV > 60%). • Valves : Les valves (mitrale, tricuspide, aortique) présentent une ouverture et une fermeture normales, sans fuite ni rétrécissement. • Péricarde : Absence d'épanchement.

La couleur (Doppler) : Elle représente le flux sanguin. Là où la plaque est présente, le passage du sang est rétréci (sténose), ce qui modifie la vitesse et la couleur du flux sur l'écran.
