After you’re first diagnosed
Combination of three or four drugs including:- Daratumumab (Darzalex)
- Bortezomib (Velcade)
- Lenalidomide (Revlimid)
- Dexamethasone
- Isatuximab-irfc (Sarclisa)
- Cyclophosphamide
If you’re eligible, your doctor might also recommend a bone marrow transplant. After first-line therapy, you’ll likely begin maintenance treatment to stay in remission.
THIRD-LINE THERAPY
After the second relapse
Depending on which drugs were used before and how long remission lasted, another combination of three drugs from the first or second line may be used. If the disease is more aggressive, doctors may add a fourth drug or move to newer options faster.
SECOND-LINE THERAPY
When myeloma relapses for the first time
Combination of three drugs, often including drugs from the list above along with one or more of:- Carfilzomib (Kyprolis)
- Ixazomib (Ninlaro)
- Elotuzumab (Empliciti)
The choice of drugs will depend on which drugs were used in the first-line therapy and how well they worked. Bone marrow transplant may also be an option.
FOURTH-LINE THERAPY
After the third relapse
Newer immunotherapies including:
- CAR T-cell therapy
- Bispecific antibodies
- Antibody-drug conjugate
Many of these target a protein in myeloma cells called BCMA.
