What is changing?
Viewing Tele Strips in Epic:
- Go to CHART REVIEW THEN the RHYTHM STRIP TAB.
- Choose the rhythm you wish to view, then click the blue hyperlink highlighted in the picture.
- if strips do not appear, go to the OnBase Patient window to view (arrow pointing to it below).
Tele will now send ALL tele strips electronically directly to the medical record. RNs will access these tele strips in Epic. Paper tele strips will no longer be printed.
Tele will continue to send routine tele strips, but this will be done electronically as well. They will send them to your patient chart at the same time increments as currently.
Strips will be measured by tele with e-calipers and will be on every applicable strip.

Documentation of Tele Strips in Epic:
- Go "Complex Assessment" or "Adult Assessments" and go to the cardiac section.
- May have to expand the documentation choices by clicking the expand button.
- Document the strip by documenting the regularity, name of rhythm, ectopy if present, ectopy frequency if present, cardiac symptoms if present, tele status and alarms if applicable. (depending on clinical condition other documentation may be neccesary- i.e. pacer info).
The RN is responsible for validating the strip after tele transmits the strip electronically.
Documentation of ALL assessments, interventions and provider notifications should also be documented!
The RN does this by documenting thier assessment of strip and documenting any other interventions or assessments needed for the patients condition at that time.

Communication and Safety Reminders
- Tele techs will still call for ALL alarms and changes to the bedside RN.
- Current communication flows and practices continue. The only new change is that the strips are sent electronically.
- When calling the tele tech for new patients/admits/transfers/change in locations, please also provide the CSN along with all other patient identifiers.
- Remember to call tel PRIOR to moving patient locations, changes in tele boxes, batteries, etc. to ensure safe patient monitoring.
- Reminder that ALL patients require a provider order to come off telemetry for any reason (bathroom privledges, scans, PT/OT, etc.). Failure to have a provider order is practicing out of scope.

It is the bedside RNs responsibility to take action for all tele alarms, changes and communications. Tele is there to assist, the RN owns the patient and patient plan of care.
