ESCAPE ROOM Secret Agent
Mission 01
THIS IS YOUR STORY: A mysterious illiness is about to happen. Can you and your team of clinical specialists identify what is causing the illiness and cure the patient. You are trapped in this room with only fragments of patient charts, lab results, and cryptic notes left behind. The key to escape lies in diagnosing the unknown illness and restoring the stability of your patient. Make sure to have your notebook handy to jot down the clues you will be collecting. If you gather all the clues, you will obtain the combination that opens the briefcase with your reward.Good luck!
You are a nurse working in the intensive care unit. You receive a post- op patient from the OR.John Smith is a 39-year-old male who had a lap colectomy performed under general anesthesia. The procedure was uneventful, and he was transferred for routine post-op monitoring due to a history of mild obstructive sleep apnea. His past medical hx includes hypertension, seasonal allergies, HLD, and GERD. He has no known allergies. Dressings are clean dry and intact. Vital Signs are as follows Temp 99.3, HR 110, RR 26, BP 136/85, O2 Sat 90% on 2 L NC, ETCO2 38, and muscle ridgity noted.

- Intubation might be necessary, hyperventilate the patient to blow of excess CO2.
- Cool the patient > 39o C, stop cooling if temperature drops < 38.5o C.
- Insert Foley
- Labs: CK, UA, coagulation studies, ABGs CMP. Repeat as needed, could be as frequent as every 6 hours for CK.
- Treat arrthymias following ACLS protocol - avoid Ca Channel blockers (may cause hyperkalemia or cardiac arrest in the presence of Dantrolene)
- Treat hyperkalemia with Calcium gluoncate or Calcium chloride for arrhtyhmis, glucose/insulin and bicarbonate can also be given. Check blood sugars hourly.
- Continue to monitor for s/sx

Dantrolene Information
- Stop triggering agent if needed
- 2.5 mg/kg Rapid IV - Weight based med.
- 60 ml of Sterile Water for reconsitituation.
- Repeat until s/sx of MH are reversed.
- Takes 90 seconds to reconstiute, mulitple people need to be assisting with this.
- Dantrolene can not be administered with Normal Saline.
- The average person could need 10-15 vials depending on patient weight.

- Onset of symtpoms can be immediate or delayed up to 4-8 hours.
- You need to activate your emergency response team to respond and bring the supplies!
