LEVEL 1: Introduction
SBAR
Situation - what's going on.Background - patient's history. Assessment - assessment of signs and symptoms (include recent set of vital signs). Recommendation - recommendation on what patient needs.
Use for escalation of care
Medical Emergency (IMT) Team
- This Team responds when there is a medical emergency called via *9999.
- Comprised of House Supervisor (RN), Security, and Respiratory Therapist.
- Patient's Primary Care Staff prepares to give SBAR to House Supervisor.
EC CODE TEAM
- This Team responds when there is an code called.
- Comprised of Physician, Nurses, Respiratory Therapist, Security, and House Supervisor (RN).
- Patient's Primary Care Staff prepares to give report using SBAR to ER code team.
- Patient's provider to give SBAR to ER provider.
- Will come with a stretcher, oxygen, and emergency medications

In addition to the crash cart, a vital sign machine is essential to obtain the most recent and accurate patient vital signs. These values are critical to share during handoff and when reporting to the IMT/EC code team to support rapid assessment and decision-making.

Wheelchair: is helpful for patient transport, but not a priority during an acute event. When the Medical Emegergy (IMT) is activated, security will arrive with a wheelchair if needed. When the EC code team is activated, a stretcher will be available to transfer patient if needed. AED: is not required if a crash cart is present. Crash carts are equipped with a Zoll monitor/defibrillator, so a separate AED is unnecessary. Nothing: Additional equipment beyond the crash cart is still needed to support patient assessment and communication during the response.

Scenario 1: A provider rushes past and urgently says, "Call a code for Room 10 now - he's not responsive!" The provider disappears into the room. You don't know if the patient is breathing or not - and you don't have time to check.
Yes! You call the EC Code Team because you are unsure if the patient is awake or breathing.
Since you are unsure if the patient is awake or breathing, it would be better to request for more support from the EC Code Team.

Scenario 2: A nurse quickly tells you, "The patient in Room 12 is having a seizure and it has already been 3 minutes. We don't have any medications for seizure here. Can you call *9999?"
Yes! You call the EC Code Team because patient additional medication and support might be needed to stop the prolonged seizure. The EC code team is comprised of a physician, respiratory therapist, house supervisor, and security.This team comes with an ER provider along with emergency medications.
Since patient may need additional medication and support to stop the prolonged seizure, it would be better to call the EC Code Team. This team comes with an ER provider along with emergency medications but IMT team does not.

Scenario 3: The provider who is evaluating your patient in room 5, came out of the room and said "Her labs do not look good and she's more tired than usual. She is stable for now but I can't send her home like this. She needs to be transfered to the ER."
Yes! Since patient is stable but not safe to go home, the House Supervisor from the Medical Emergency (IMT) Team can take patient to the ER for further evaluation.
Since patient is currently stable and not in distress, calling the Medical Emergency (IMT) Team first would be ideal. The House Supervisor can determine if more support is needed.
