Segundo a DGS (2011), 95% das LPP são evitáveis através de intervenções de identificação precoce, o que torna o conhecimento dos sinais associados ao desenvolvimento de LPP a chave para diminuir a sua prevalência.
Gonçalinho & Costa (2025) destacam que a prevenção de LPP em doentes críticos passa pelo reposicionamento frequente, utilização de colchões e almofadas que distribuem a pressão, inspeção diária da pele e proteção da mesma quando em contacto com dispositivos médicos. A nutrição de qualidade e a formação contínua da equipa de enfermagem destacaram-se também como essenciais para manter a saúde da pele, bem como a aplicação correta dos protocolos de prevenção.

PREVENÇÃO IFC
A prevenção da IFC em doentes críticos deve ser multifacetada, envolvendo cuidados pré-operatórios, profilaxia antibiótica, técnicas assépticas rigorosas, monitorização contínua e gestão dos fatores de risco (Zhou et al., 2024).
A norma nº020/2015 da DGS atualizada em 2022 apresenta como feixe de intervenções para a prevenção da IFC as seguintes boas práticas, o banho pré operatório com cloroexidina 2-4% na noite anterior e no dia da cirurgia, a profilaxia antibiótica cirúrgica, de acordo com protocolos estabelecidos que tenham em conta o tipo de cirurgia e o risco de infeção e a adoção de técnicas assépticas rigorosas e auditorias continuas revelam-se também fundamentais para a avaliação da adesão às práticas de prevenção e identificação de áreas de melhoria
Um estudo retrospetivo conduzido nas UCIs portuguesas entre janeiro de 2017 e junho de 2023 revelou que a prevalência global de LPPs em contexto de UCI foi de 15,2%, a incidência cumulativa ao longo dos seis anos foi de 13,1% e as LPP mais comuns foram de grau II (47%) e I (34%) (Vieira, Mostardinha, & Alves, 2024).
De acordo com ao SEMICYUC (2017), a incidência LPP em pacientes críticos não deve ultrapassar os 5%, sendo esta percentagem considerada um indicador da qualidade dos cuidados prestados na UCI.
NA PRÁTICA?
Nas LPP é responsável pela realização de avaliações de risco sistemáticas no período pré e pós-operatório, utilizando ferramentas validadas que permitem identificar precocemente os doentes vulneráveis (Galvin & Curley, 2012; Kaşıkçı, Aksoy, & Ay, 2018). Estas avaliações possibilitam a implementação de intervenções adequadas, medidas essenciais para a redistribuição da pressão e prevenção de lesões (Lyder, 2006).
Exerce um papel de liderança clínica e educativa, orientando equipas de enfermagem na aplicação de técnicas corretas de mobilização e cuidados cutâneos. A formação contínua é um elemento-chave, uma vez que o conhecimento e a atitude dos profissionais estão diretamente associados à eficácia das medidas preventivas (Liang et al., 2024; Zencir et al., 2025).
CONCLUSÕES
Em síntese, a análise desenvolvida neste trabalho permite concluir que o controlo das IACS depende fortemente da atuação qualificada do EEMC. A articulação entre a prática, investigação e formação contínua constitui o alicerce para a excelência dos cuidados e melhoria contínua dos resultados em saúde, reafirmando o compromisso ético e científico da enfermagem na prestação de cuidados de excelência salientando ainda a importância da formação e valorização dos EEMC como um investimento na qualidade, sustentabilidade e segurança dos cuidados de saúde..

A atuação do EEEMC segue uma abordagem sistemática centrada na avaliação de risco, prevenção e vigilância clínica contínua. Na IFC, avalia fatores como tipo e duração da cirurgia, profilaxia antibiótica, controlo glicémico, normotermia e oxigenação, conforme o bundle da DGS (2022) e a OMS (2018)

PAPEL DO EEEMC NA PREVENÇÃO DAS LPP
As orientações do CDC (2024) para a prevenção de LPP em UCI recomendam a inspeção diária da pele, especialmente em áreas de risco como sacro, calcanhares e cotovelos, o uso da escala de Braden para identificar doentes vulneráveis e orientar as intervenções e a adoção de cuidados preventivos, como reposicionamento frequente, superfícies de suporte adequadas e barreiras protetoras cutâneas.
A implementação destas medidas tem o suporte de meta-análises que validam a redução de incidência em até 60% com aplicação deste tipo de bundles (Lavallée et al., 2021; Tayyib & Coyer, 2021).

PREVENÇÃO IFC
A prevenção da IFC em doentes críticos deve ser multifacetada, envolvendo cuidados pré-operatórios, profilaxia antibiótica, técnicas assépticas rigorosas, monitorização contínua e gestão dos fatores de risco (Zhou et al., 2024).
A norma nº020/2015 da DGS atualizada em 2022 apresenta como feixe de intervenções para a prevenção da IFC as seguintes boas práticas, o banho pré operatório com cloroexidina 2-4% na noite anterior e no dia da cirurgia, a profilaxia antibiótica cirúrgica, de acordo com protocolos estabelecidos que tenham em conta o tipo de cirurgia e o risco de infeção e a adoção de técnicas assépticas rigorosas e auditorias continuas revelam-se também fundamentais para a avaliação da adesão às práticas de prevenção e identificação de áreas de melhoria
Um estudo retrospetivo conduzido nas UCIs portuguesas entre janeiro de 2017 e junho de 2023 revelou que a prevalência global de LPPs em contexto de UCI foi de 15,2%, a incidência cumulativa ao longo dos seis anos foi de 13,1% e as LPP mais comuns foram de grau II (47%) e I (34%) (Vieira, Mostardinha, & Alves, 2024).
De acordo com ao SEMICYUC (2017), a incidência LPP em pacientes críticos não deve ultrapassar os 5%, sendo esta percentagem considerada um indicador da qualidade dos cuidados prestados na UCI.
NA PRÁTICA?
Nas LPP é responsável pela realização de avaliações de risco sistemáticas no período pré e pós-operatório, utilizando ferramentas validadas que permitem identificar precocemente os doentes vulneráveis (Galvin & Curley, 2012; Kaşıkçı, Aksoy, & Ay, 2018). Estas avaliações possibilitam a implementação de intervenções adequadas, medidas essenciais para a redistribuição da pressão e prevenção de lesões (Lyder, 2006).
Exerce um papel de liderança clínica e educativa, orientando equipas de enfermagem na aplicação de técnicas corretas de mobilização e cuidados cutâneos. A formação contínua é um elemento-chave, uma vez que o conhecimento e a atitude dos profissionais estão diretamente associados à eficácia das medidas preventivas (Liang et al., 2024; Zencir et al., 2025).
CONCLUSÕES
Em síntese, a análise desenvolvida neste trabalho permite concluir que o controlo das IACS depende fortemente da atuação qualificada do EEMC. A articulação entre a prática, investigação e formação contínua constitui o alicerce para a excelência dos cuidados e melhoria contínua dos resultados em saúde, reafirmando o compromisso ético e científico da enfermagem na prestação de cuidados de excelência salientando ainda a importância da formação e valorização dos EEMC como um investimento na qualidade, sustentabilidade e segurança dos cuidados de saúde..

PAPEL DO EEEMC NA PREVENÇÃO DAS LPP
As orientações do CDC (2024) para a prevenção de LPP em UCI recomendam a inspeção diária da pele, especialmente em áreas de risco como sacro, calcanhares e cotovelos, o uso da escala de Braden para identificar doentes vulneráveis e orientar as intervenções e a adoção de cuidados preventivos, como reposicionamento frequente, superfícies de suporte adequadas e barreiras protetoras cutâneas.
A implementação destas medidas tem o suporte de meta-análises que validam a redução de incidência em até 60% com aplicação deste tipo de bundles (Lavallée et al., 2021; Tayyib & Coyer, 2021).
