Questions & After Course

  • During this online orientation course:
    • Take note of any questions you have (bring these with you to the in-person Periop Orientation session listed in your welcome email)
    • Pay attention to the animated icons throughout the module for more information available about the topic -- either in this module or in another, more extensive module in the Learning Exchange (LMS)
    • Resources are linked here and accessed by clicking the paperclip icon
  • At the in-person Periop orientation session, we will:
    • Cover some additional content that all Periop staff receive
    • Give you helpful badge buddies

    • Have you demonstrate select skills covered in the online module
  • After the in-person session:
    • Complete other online orientation modules assigned (several topics covered in this course are covered in-depth in other LMS courses; those courses do NOT need to be complete before the in-person Periop orientation session)

Quarterly M&M

  • Usually cases are scheduled to start at 0730 (meaning: patient in the room)
  • Perioperative Services has “late start” on Wednesdays and Fridays (meaning: cases are scheduled to start at 0800 instead)
    • Allows time for in-services and meetings
    • On the first Friday of the month quarterly (in March, June, September, & December), cases are scheduled to start at 0900
    • Multidisciplinary teams of Perioperative Services attend the Morbidity & Mortality (MMI) conference in Langford Auditorium
      • Focuses on topics to reduce medical errors in our workplace
      • Attendance required if you are scheduled to work
    • Steps
      • First, sign up at the CE site
      • Then, during the meeting, text the conference code to (855) 776-6263 to record your attendance

      eOR Board & Command Center/Hub

      • The assignment board is where staff names are listed according to which OR they will be working in that day
      • The Command Center/Hub is where all decisions are made about case placement and operations for the day

      OR Flightboard

      • The Flightboard has basic information about the current patient

      Standard Precautions

      • A set of infection control practices to prevent the transmission of infectious disease based on the assumption that all people may be potential carriers of disease and regardless of their apparent health status
      • Includes
        • Hand hygiene (covered in the next slide)
        • Cleaning reusable equipment
        • Proper waste disposal
        • Appropriate PPE for the situation
        • Appropriate attire

      Hand Hygiene

      • Use proper technique to reduce cross-contamination by hands onto other surfaces
      • If hands are not visibly soiled, you may use a hospital approved waterless hand sanitizer
      • Fingernails must be short, clean, and healthy (in good condition)
      • Nail polish (regular polish) must not be chipped
      • Long natural fingernails as well as artificial nails (acrylics, overlays, bonding which includes dip polish, tips, extensions, tapes, inlays, and wraps) are prohibited

      Identification Badges

      Jewelry

      • ID badges must have name, title, and picture clearly visible and worn in clear sight above the waist
      • Other people present in your room must wear their badge too (meaning: you should know who is in the OR with your patient and if they are supposed to be there)
      • Jewelry that cannot be confined or contained in scrub attire/PPE should not be worn in the semi-restricted and restricted areas
      • Microorganisms thrive under jewelry

      Traffic Zones

      Surgical Attire & PPE

      Surgical Attire: Scrubs

      • VUMC launders and issues scrub sets and scrub jackets; wear personal attire to work then change into a freshly-laundered set of scrubs
        • Healthcare laundry facilities must launder to standards not met with home laundering equipment
        • These are the only attire approved to be worn in the OR (a bunny suit that completely covers personal attire may be worn by those briefly visiting to do essential tasks)
        • Obtain and return these scrubs from the machines in your work area
      • The department gets charged if scrubs are not returned within three days — return these promptly
      • A short-sleeve shirt may be worn under scrub tops (not a long-sleeve)

      Surgical Attire: Jackets

      • Fuzzy jackets attract lint and are not permitted in semi-restricted or restricted area

      Personal Protective Equipment (PPE)

      • Head coverings
        • Confine all hair, scalp skin, and facial hair — hair shedding is a risk of SSI
        • Cloth hats may be worn but must be laundered between workdays (daily if necessary)
      • Masks
        • Completely cover the mouth, nose, and chin snugly
        • Do not dangle from the neck
      • Eye Protection
        • Wear approved eyewear with side protection when patient in the OR or otherwise providing direct patient care
        • Regulatory requirement
      • Shoe covers/boots
        • Worn in instances when gross contamination is anticipated
        • Replace when soiled or leaving the room
      • Exam gloves
        • Worn only when performing dirty tasks (not when performing clean tasks)

      Personal Items

      • It is best to take no personal items (like backpacks) to the OR
      • Personal items (like backpacks) that are taken into the semi-restricted or restricted areas:
        • Must be cleaned with a low-level disinfectant
        • Must be confined to a clear plastic bag (never place personal items directly on the floor which is always considered contaminated)
      • Cell phones may be brought into the OR if needed for job performance
        • Must be cleaned with a low-level disinfectant before and after being brought into the perioperative setting
        • No pictures of patients or private health information (PHI)
        • Use vibrate or silent mode to minimize distractions and excess noise in the room — alarm fatigue

      Description of the M&M Conference

      The conference consists of presentations of cases in which outcomes were not expected and/or not desirable or presentations of relevant quality-related issues that impact providers in the perioperative spaces followed by discussion of strengths and weaknesses of the patient care provided, institutional support provided, relevant research evidence, and strategies for improvement, with an emphasis on quality improvement. The goal of the conference and associated quality improvement efforts is a system of health care delivery that is error-free.
