A FEW DOCUMENTATION HOUSEKEEPING POINTS:

  • Child refusals, transition concerns, and sessions ended early by child now have required intervention and support attempts to be documented.
    • There is a basic formula for the wording associated with documenting these interactions.
  • There is a basic formula for cancellations as well.
  • Please be aware that N/A can only be used in "no visitors" section or "children not attended" section.
    • LANGUAGE TO USE IF NO INTERVENTIONS OCCURRED is as follows: “Growth Works does not have anything to report related to this section".
  • Session report intervention documentation now has a formula:
    • If an intervention occurs, staff should report the intervention during the visit and the follow-up conversation with the visiting parent after the visit.
      • DURING A VISIT: report interaction or statement that required intervention, state that the case manager intervened and redirected the conversation. Include parents reaction to intervention.
      • AFTER THE VISIT: report what Case Manager discussed with visiting parent and if the parent was not receptive & interactions that followed.
