Focus: Increase household income with a holistic approach Success Measure: - Each household increases income (wages and/or benefits) - Household exits to PH and do not return to homelessness Immediate Action: - Determine partner capacity - Complete system mapping (needs, gaps, resources - Referral data mapping CDSS Support - Connection to state partners for economic development

Focus: Bringing managers in closer to the work. - Communicating up to the Board/new nuance about the work. - Aging out of existing housing. Services to transition older adults. - Tailored wrap-around services. Master Plan for Aging. - Older adult CoC. Success Measures: - A Draft CoC - Defining pathways & partnerships - No wrong door policy
Immediate Action: - Bring ASPC CoC to HSOC re Older adults and homeless services CDSS Support - Older adults specific TA/resources - Training on implementation supports - Build into funding requirements (continuity)
