ESCAPE ROOM Secret Agent

THIS IS YOUR STORY: Welcome to Takeda’s facility, where precision, compliance, and teamwork are everything. A mysterious system glitch has locked down the final batch release process. The only way to restore operations is to crack the Quality Code — a multi-layered puzzle hidden across the facility’s core quality checkpoints, and unlock the briefcase to win! Your team of quality experts must navigate through three immersive missions, each representing a critical component of Takeda’s commitment to excellence.

Mission 01: TAKEDA

Mission 01

A deviation alert has triggered a lockdown in the Quality Control system. But this time, it’s not about temperature logs — it’s about team knowledge gaps. To restore system access, your team must prove its understanding of Takeda’s quality principles, systems, and culture.

Your first task is to find the hidden clue in our WQW Daily Gazette....

Mission 02: THe HIDDen PATTERN

Your Quality Task Force has intercepted four confidential incident reports from different production sites. At first glance, they appear to be ordinary deviation summaries — but your inside contact warns you otherwise. A mysterious figure known only as “The Calibrator” has tampered with these reports. He believes true quality means seeing beyond compliance — and he’s left behind an encrypted message for those sharp enough to uncover it. Rumor has it that the key to cracking his code lies not in the numbers, but in the patterns of failure hidden within the incidents themselves.

Mission 02

It seems that someone has hide a secret code in the company's incident files.....
Determine what the root case of each report is: equipment, material, human, method. Once you've determind the root causes, use the first letter of the root case to form a four-letter- code that you will submit to move on to the next mission

Mission 03: Connection Matrix

The part of our system responsible for linking data, people, and processes — has crashed. The AI can no longer recognize meaningful relationships between key elements in the manufacturing process. Your task: Restore the connections by matching pairs that belong together, not because they look alike, but because they work together. When all connections are restored, the system will reveal the next part of the code.

Mission 03

Task: Restore the connections by matching pairs that belong together, not because they look alike, but because they work together (hint: think quality vs innovation)

Congratulations! You now have your last secret number to open the briefcase, write it down and move forward

Incident Report 2

Classification: CONFIDENTIAL Origin: Production Site Charlie Report ID: CHR-IR-401 Timestamp: 09:37 hrs Incident Type: Incoming Material Defect Summary: A new batch of polymer feedstock (Lot #47B) showed inconsistent viscosity during testing. Production team proceeded, assuming normal variation, but later reported defective mold fill and non-uniform parts. Review of the supplier's Certificate of Analysis indicated deviation in polymerization consistency. A follow-up audit revealed procedural changes at the supplier’s plant, not communicated to quality control.

Incident Report 2

Classification: CONFIDENTIAL Origin: Production Site Bravo Report ID: BRV-IR-284 Timestamp: 14:52 hrs Incident Type: Equipment Vibration / Product Loss Summary: Conveyor Line 3 began vibrating abnormally during packaging operations, leading to alignment issues and spillage. The maintenance log confirmed a motor replacement had occurred the previous day. Inspection found that one of the coupling bolts had not been torqued to spec, allowing shaft misalignment under operational load. There was no indication of sabotage or misapplication — rather, improper reassembly during maintenance.

Riddle

I’m why we care, the center of our aim.I’m built with honesty, not fortune or fame. I’m how the world sees what we do. I’m what thrives when the first three are true. What are the four that guide our way? What am I?

Incident Report 1

Classification: CONFIDENTIAL Origin: Production Site Alpha Report ID: ALP-IR-309 Timestamp: 02:13 hrs Incident Type: Emergency Shutdown Summary: Line 4 experienced a full emergency shutdown after overheating in the thermal chamber. The system failed to maintain safe operating temperature limits during the overnight shift. Investigation revealed the operator had manually bypassed the automatic interlock system, which was delaying due to a lagging temperature sensor. When interviewed, the operator admitted to overriding the system to avoid delays, citing pressure to meet output quotas. No mechanical or system faults were identified.

Incident Report 4

Classification: CONFIDENTIAL Origin: Production Site Delta Report ID: DLT-IR-552 Timestamp: 17:06 hrs Incident Type: Calibration Procedure Deviation Summary: During routine audit, three pressure gauges were found significantly out of calibration tolerance. Review of calibration records showed technicians had followed the latest SOP revision exactly. However, the SOP itself — last updated 6 weeks ago — had a critical verification step removed due to a formatting error during document editing. This error was not caught during the approval process.
