Welcome, PMHNPs-in-training! You’ve just arrived for your first clinical rotation at Willow House Wellness Center. This is a clinic that serves rural and urban patients; it’s located at the edge of town. In addition, the Center serves patients without insurance. Willow House was built in the mid-1800s by a local town founder. Its name derives from the fact that the house was built in a clearing surrounded by willow trees. The town founder was a proponent of inclusivity and health and left the house to a foundation with the understanding that the foundation would establish a wellness center therein.

The Situation

Eager to get started with your first clinical Willow House Wellness Center, you arrive at the Center a little early. You greet the receptionist and walk through to the locker room where you hang up your bag, put on your white coat, then get ready to start the day. Next, you walk down a short hallway to work room to log onto the computer system. Click on the ENTER button below to complete your sign-on.

The Situation

The moment you hit ENTER, the doors in the clinic bang closed and lock with a loud 'click.' An alarm sounds along with a red flashing light and an announcement comes across the PA system: Emergency Diagnostic Protocol Initiated. Clinical Competency Required To Exit.

Throughout the Clinical Competency Challenge, you will answer questions pertaining to assessing a patient, including general assessment, mental health assessment, and assessment various body systems. Sometimes, your answers will get you out of a locked room; other times, your answer will allow you to proceed along a hallway to the next room. There is no time limit for this challenge. All rightie, you think. That’s probably true; I can’t assume that someone believes something just because of their culture or religion or anything. I have to ask them. And, moving on to #3.

Ball et al. recommend being flexible as you establish a partnership with a patient, as well as negotiating roles when necessary, and stressing that you're working together to address health problems. (p. 11) Three down, one to go.

As you answer the last question correctly, the door of the workroom opens, and you step to the doorway. There before you is a long hallway in which you can see that there are offices on your left side and a few exam rooms ahead, in the left corner. You cannot see around the corner but can see that there are obstacles in the middle of the hallway that you will have to get over or around to move on. You’ve arrived in Anxiety Alley. Slowly, not knowing what to expect, you move into the Alley and reach the first obstacle, a pair of exam tables blocking the way. On one of the tables is an iPad with a Post-It note that says, “Tap the iPad to begin.” So, you tap the iPad, and a question pops up which you hope will help you get over or around this first obstacle. The question is:

The iPad flashes green and you breathe a sigh of relief but before you can even wonder how you’ll get over or around the exam tables, they recede into the floor, opening your path. Now, for the second obstacle which, on closer inspection, appears to be an old console television stood in the middle of the hallway. There is a note on the TV, too. It says, “Turn me on.” Well, this TV looks nothing like the TVs you’re used to but there is a button on the right side so you push it and the screen flickers to life. There is a question and this time, the answer options are numbers. Your directions are to answer the question by turning the channel dial to the number associated with the answer options. Blowing out a breath and looking nervously at the TV to make sure you know which dial the directions mean, you read the question. Click on the QUESTION button below to open the question.

You turn the dial to the answer option you believe to be correct and to your surprise, the console TV disappears into the floor, just like the exam tables, giving you free access to the last bit of hallway before the corner. And when you turn that corner, you are startled to see a life-sized man standing there wearing black armor: The Black Knight from Monty Python and the Holy Grail. Click on the knight to find out what you need to do to get past him.

“Hello?” you say tentatively.The knight doesn’t move. He stands with the tip of his sword on the hardwood floor, both hands clasped around the hilt. “OK,” you mutter. “Maybe it’s a statue.” So, you move forward, trying to figure out how to get past the statue, when he raises his sword and says ominously, “None shall pass…” “I have to get through here,” you say. “I have to answer a question so that I can move on in this challenge.” “None shall pass.” Well, now you think that this statue has to be an automated robot or something, so you say, “Sorry. I have to pass,” and duck under his arm to try the door knob. Locked. When you turn back to the knight, he’s not there but there is a piece of paper fluttering to the floor with the next challenge question. Click on the knight again to see the question he left for you.

The first letter of your scrambled escape code is... P. And phew!, you made it into the next challenge room: Neuro Niche. In this room, you will answer questions about basic neurological assessment. You’re a little nervous because you haven’t had this content in your assessment course yet, but you figure you assess patients’ neuro status all the time!, how could this be different? That wasn’t so hard. You look around the room and see an envelope taped to the wall over the eye chart. Inside the envelope is this question:

You hear the sound of a lock clicking and realize you got the answer right. There is another envelope taped on your side of the door and inside is the second letter of your escape code: S. The door swings open and you’re back in the hallway. The only option is a door to the next room, but there is a gate barring the doorway. A plaque is hanging on the gate; your next challenge question.

The Depression Den

The gate rolls upward with a clanking sound, and you are able to enter the next challenge room: Depression Den. The room is dark and a little dank; kind of like you’d expect a depression room to feel. A single table stands in the middle of the room and on it, there is another iPad with the same Post-It note: “Tap the iPad to begin.” A question pops up with four possible answers. Click on the iPad to move on to the question.

Again, relief as the iPad turns green then gives you the third letter of your escape code: A. Another door opens, this one leading to the waiting room. You can see that the other entrance to the waiting room is barred and look around for a question to answer to keep moving forward. Click on the iPad key to go to the waiting room.

The gate barring the other waiting room door swings open and you emerge from the waiting room and start walking up the stairs. At the top of the stairs, you find yourself in an enclosed landing. There is a door on the right side, a door on the left side. And there is another iPad with the usual note. You tap the iPad onthe + sign and a question pops up.

The door to your right opens, leading into a hallway that is long but not as long as the one downstairs. The door directly in front of you, with a sign that says, “Pulmonary Parlor,” is locked. There is a doorbell on the right side of the door, so you push it, and an envelope appears from under the door. In the envelope is the question to get the key to this door and you are instructed to say the answer out loud in order to obtain the key.

You unlock the door and enter Pulmonary Parlor which you find filled with white balloons hanging from the ceiling. Thinking back to your pathophysiology class, you wonder if these balloons are supposed to represent the alveoli in the lungs. The door closes behind you and you note that there is a message written on a white board hung on the door. So, you start looking at the balloons, utterly certain that you’ve looked at more than one of them more than once. You finally find the first question and use the pen in the pocket of your white lab coat to pop the balloon and retrieve the question. Click on this button to read the question.

Find the next 2 questions by examining the balloons. The questions are INSIDE 2 of the balloons.

As you answer this second question correctly, a hidden door opens; you hear it but cannot see it due to the remaining balloons so have to bat them aside as you walk across the Pulmonary Parlor to the now-open door. Hanging in the doorway is a message with your next escape code letter: T.

Welcome to Cardiac Corner. There are several small tables placed around the room, each of them with an iPad. The directions on the first table tell you that when you answer the question on this iPad correctly, the next iPad will ‘ping’ and light up, telling you where to go. OK, you think. Let’s do this. I remember my cardiac! You tap the screen of the first iPad, and the question appears:

‘Ping’ number 4 sounds in the room, near the door that you came in, which is now, mysteriously closed and locked. The next question is:

There is a loud ‘bong’ this time and the iPad you just used gives you the next letter in your escape code: N. A panel in the wall slides open, allowing you to leave Cardiac Corner and enter another long hallway, this one called Bipolar Byway. You can see that there is a door at the end of the hallway and figure there is at least one (1) question to answer, but there may be others, so you proceed slowly. After a few steps, a panel in the wall to your right slides open to reveal a monitor. The directions tell you that it’s a touch-screen monitor so you can select your answer by touching the monitor. Click on the button below to open this next challenge question.

The door at the end of Bipolar Byway opens a crack and when you pull the door the rest of the way open, you see another note hanging in the doorway. Next escape code letter: I. You put the message in the pocket of your white lab coat and move into a smaller hallway with a door on the opposite wall that is closed. Against the wall, there is a table with, yes, another iPad. You tap the screen, and a general assessment pops up. Phew! Those specialized questions over material you haven’t had yet were tough. Hopefully, this one will be a breeze!

A client presenting with new-onset anxiety, weight loss, and tremors may have symptoms related to endocrine dysfunction, most commonly hyperthyroidism. The thyroid gland regulates metabolism. Excess thyroid hormone (thyroxine) increases metabolic activity, leading to weight loss despite normal or increased appetite, fine tremors, heat intolerance, and anxiety or nervousness due to overstimulation of the sympathetic nervous system. A. Cardiovascular – Hyperthyroidism can cause palpitations or tachycardia, but the constellation of anxiety, tremors, and weight loss points more directly to endocrine dysfunction. B. Neurological – Tremors can be neurological, but paired with weight loss and anxiety, they are more characteristic of thyroid disease. D. Gastrointestinal – Weight loss can be GI-related, but GI disorders typically present with nausea, vomiting, diarrhea, or malabsorption rather than tremors and anxiety. Therefore, the endocrine system (especially the thyroid) should be evaluated in this case.

A drawer in the iPad table pops out. Inside the drawer, there are two things: a piece of paper and a key. The paper has your next letter for the escape code: T, and the key unlocks the door to the next challenge, The Stress Salon. Click on the key to the right to move on into the Stress Salon.

You have to walk along a hallway to get to the Stress Salon, but fortunately, there is nothing barring your path, no questions to answer. What a relief! In the Stress Salon, there is a large flat screen monitor with a welcome greeting. Then, you’re directed to the three tables below the monitor, each of which has a question for you to answer. Once you answer all challenge questions correctly, you will be given the next letter of your escape code then directions for completing the challenge. You’re nearly there, the message tells you. The first question is on the center table, on the iPad. You tap the screen and the question appears.

This iPad gives you the next letter for your escape code: E, then a panel slides open and you’re able to walk along a hallway to the staircase where you go downstairs, for that’s your only option. Let's make our way downstairs... If you dare... Click on the stairway to move on.

Downstairs, your only option is to walk along another hallway, this one seems to go diagonally and you think you’re headed to the back of the house/clinic. The hallway opens into the break room where everyone eats lunch and drinks their coffee. There is a single door leading to the outside, probably to the verandah that goes around the house. And on the door, there is another iPad. “I wish I’d bought stock in Apple,” you say as you walk over to the door. The directions on the screen tell you to enter the escape code. There is a pad of paper and pencil on the break room table so that you can write down the letters you’ve gotten: P, S, A, T, N, I, T, E, then unscramble them and enter the escape code on the iPad. You come up with a couple of options but there are always letters left over. Finally, you realize the code and enter it on the iPad. (Enter the code below.)
