Provider General Information Guidelines
Provider general information must be complete, accurate and verifiable. Providers must include their legal name, complete legal address and other contact details. Providers can also include display information such as a preferred name, and a current location. The reason we allow display information, is that a provider may want to be called by their middle, preferred, or nickname. Also, a provider might currently be staying somewhere other than their legal address- such as a college campus away from home. Individuals who attempt to enroll with fake names, celebrity names, gibberish, inappropriate words or nonsense must be rejected and closed.
