This should be the formal name that you would call the person you are writing the note to. For Example: Dear Mrs. Smith,

End your letter formally. You can use sincerely, best, best regards, or many thanks and then your name.
Tips
- Send as soon as possible.
- Be positive and sincere.
- Personalize each letter.
- Keep it brief.
- Proofread!
- Give specific details about what you are thanking the person for.
- A handwritten and mailed note is more personal, but you can send it as an email if necessary.
