- You pay a smaller premium than the Traditional Plan, but pay more upfront on healthcare costs with a high deductible.
- You get money in an HSA or HRA for health-related expenses to offset your higher deductible.
- Your Out-of-Pocket Maximum includes what you've paid in Deductible.
- The STAR HSA plan has extra preventive services paid at 100% compared to the other plans. There are also several Preventive Medications covered before the deductible. See page 19 of the

*One individual cannot meet more than $4,000 out of pocket on their own. This means once one individual has met their $4,000 out of pocket maximum, they're covered, and in-network expenses are covered 100%.

- You have the option to add a FLEX$ Spending Account (FSA) and/or a Dependent Care Flex.

- Similar to the STAR HSA Plan with a higher deductible, lower coinsurance, but fewer covered benefits and medications. See the Benefits Guide and Consumer Plus Covered Drug List for what is covered
- You pay a low premium (or no premium on the Summit Network), and your employer puts more money into an HSA or HRA than the STAR HSA Plan to offset a higher limits
- You can participate in wellness programs, including Healthy Utah testing sessions; however, you’re not eligible for rebates
- Your out-of-pocket maximum includes what you've paid towards your deductible.
- Preventive services are still covered at 100% before deductible

PEHP offers life and accident insurance plans to ensure your loved-ones are covered in the event of your death or disability. You can purchase up to $500,000 of life insurance for both you and your spouse, and up to $15,000 of life insurance for your dependents. You can also purchase an Accidentally Death and Dismemberment (AD&D) policy up to $250,000.
