Dos and Don'ts
DON'T - Use the same note for more than one session
DO - Create a new session note for each session
DO - Individualize the note for that specific client's session
DON'T - Use a template for the note
To be in compliance with CSD recommendations, templates should not be used as each session note needs to be individualized to the client and their specific treatment plan
Things to Remember
- Graph and End Session and begin session note during last 15 minutes of session
- Use ABC Method for Narrative Section (3-5 Targets needed per note)
- Target 1 (Structure)
- Sentence 1 - What happened before the behavior?
- Sentence 2 - Independent behavior and errored behavior with the prompt
- Sentence 3 - What happended after the behavior?
- Target 2 (Example for Skills Acquisition Target)
- When working on imitation for touching nose, the BT said "touch your nose" while touching their nose. Client independently responded 4/5 trials and required a gesture prompt 1/5 trials. BS provided vocal praise and fidget toy as reinforcement.
- Target 3 (Example for Behavior Reduction Target)
- When asked to complete a math worksheet, client engaged in 4 instances of screaming. BS provided a full verbal prompt to ask for a break.
