  • The content of the transition plan should be developed with the Young Person and their supports as appropriate.
  • Do not copy and paste from the categories, but instead use them as a starting off point for developing individualized goals and activities with your youth.
  • Focus on the Objectives and Activities. There are some ideas for "Youth's Current Abilities" and "Areas for Growth", but the most important focus is on the objectives and activities.
  • Remember to have clear timeframes on the activities and when you plan to complete them. Some of the examples include timeframes, but for the sake of completing this document, not every example has a timeframe attached. **anothe reason to be careful with copying and pasting :)
  • This is a document to support you in thinking of creative and innovative activities. If you have an activity you find effective and you'd like to add it, please feel free to do so, or ask a DCS team member to add it for you.
