Onboarding Checklist: Step by Step Onboarding Plan

Step 1: Preparing for the Home Health Nurse or Aide's First Day

✔ Create a welcome plan – Decide who will give the tour and introduce routines.

✔ Organize key documents – Care plans, emergency contacts, medication schedules.

✔ Set up a trial period – A few shifts where the nurse or aide gradually takes on responsibilities.

(This may not be an official trial period through the agency, but rather an opportunity for both the provider and family to ensure a good fit before fully transitioning into the role.)

✔ Review daily responsibilities – Feeding, medications, therapies.

✔ Clarify communication preferences – Daily updates, logs, or check-ins.

✔ Discuss boundaries & respect – How to handle family routines & privacy.

Tip: Onboarding doesn’t stop after the first day—ongoing communication ensures a strong, lasting relationship.
