Overview of Indicator 14- Identification & Purpose

  • The State Performance Plan (SPP) describes how states are improving educational outcomes for students with disabilities and complying with the IDEA.
  • Indicator 14 is one of 17 SPP Indicators that have been developed by the PA Department of Education.
  • The purpose of Indicator 14 is to collect information that will inform program improvements which will result in better post-school outcomes for youth with disabilities.

What is a SAAFP?

  • Summary of Academic Achievement and Functional Performance.
  • The LEA is required to provide this to each learner with an IEP who is exiting high school. ​
  • Purpose of SAAFP: The SAAFP is intended to be a student-centered document that summarizes individual abilities, skills, needs, and limitations. Its purpose is to provide recommendations to learners that will support their successful transition to adult living, learning, and working.​

Contact the Disabilities Services Office at the college or university the learner is interested in attending. This is to obtain needed instructional accommodations.

Mark Your Calendars!

  • Your SAAFP will be dated according to their graduation time.
    • End of Year, Summer Sessions, and ESY- June 6, 2025
  • Case Managers will notify their AA in JIRA, creating a NEW isue {Scheduled (Meeting/Invite/NOREP)} , by the following dates:
    • End of Year and ESY Grads- By 6/13/2025. These can be sent no sooner than June 9, 2025, but must be sent to your AA by June 13, 2025
  • Summer Grads- Case Manager sends the drafts of the SAAFP and GRAD NOREP (or name of the learner) to supervisor at the end of their school year. After successful completion of the Summer Session, Supervisor then finalizes the SAAFP, and sends documents to AA (AA will finalize the NOEP).
  • DO NOT Finalize your NOREPs! Your AAs will adjust DATES accordingly.

Steps to send Grad NOREP and SAAFP in JIRA
  • In JIRA: Go to IEP Compliance to create a NEW issue for your learner.
  • In forms, you will locate "Scheduled (Meeting/Invite/NOREP ONLY)
  • In the learner's name section add "Grad NOREP/SAAFP" to the end.
  • Meeting type is "NOREP ONLY"
  • In the TEXT box include: "Grad NOREP/SAAFP are ready"
  • You MUST then ASSIGN to YOUR AA

Summary of Student Performance

Academic Achievement: Gather information from the learner’s IEP and progress reports to summarize the learner’s abilities, skills, needs, and limitations. Include reading, math, writing, and any other related academic area​ Accommodations: Include the effective accommodations, modifications, assistive technology, and instructional strategies that have been utilized to assist in the learner’s educational program. *Section VI of the IEP (Specially Designed Instruction)​

Recommendations: Provide recommendations and supports that will support a successful transition to adult living, learning, working, and community​

Functional Performance: Provide a summary of the learner's functional performance to include their post-school goals. The Accommodations will have no long term supports. Recommend strategies to successfully achieve their post-school goals.

Recommendations to Assist in Meeting Post-Secondary Goals

Post-Secondary Education & Training/ Employment/ Independent Living: Completed this section summarizing the learner's post-secondary goal, according to IEP and Transition goals. Office/Agency Contact Information: Provides the learner with WHO they may contact to complete recommendations in the section, “Next Step” & "Reason for Contact"

Next Step & Reason for Contact: The recommendations should answer questions “what do I do next?” Include suggestions for accommodations to enhance access in a post-secondary environment​. Include the reason for the contact.

Student Perspective

This sections helps to promote self-advocacy and the development of an understanding of their disability and the impact it may have on their post-secondary activities. Here the learner has the opportunity to reflect on what supports and services helped them to be successful during their high school experience. The learner may also discuss what supports and/or services will be needed in their future. ​ * Case managers are encouraged to complete this section with their learners. The learner may utilize this document for their post secondary endeavors.
