Using Wound Drainage Systems

Before emptying a drain, INSPECT the system to ensure proper functioning.

Inspect:

• Insertion Site - for redness, drainage, and verify suture(s) intact
• Tubing and reservoir - for presence and color of drainage
• Patency of tubing - is it draining or clogged?
• Airtight connections - does bulb stay compressed when stopper is closed?
• Leaks or kinks in system

Emptying a JP Drain

• Hand Hygiene and gloves

• Prepare collection device (with measurements)

• Release stopper and allow JP bulb to fill with air

• Pour drain bulb contents into collection device
• Note amount and characteristics of drainage (e.g. serous, serosanguinous, sanguinous, purulent).

• Reactivate suction by compressing JP bulb and replace stopper.

• Remove gloves, perform hand hygiene, and document.

Home care: Patient education

• Empty drain every 6 hours.

• Keep a record of output (write down amount each time emptied).

• Drainage should decrease over time. The output determines when the drain can be removed.

• Cover drain site with waterproof dressing when showering.

• Call clinic if there is redness or leaking from drain site, increase in pain, change in output (increased volume or if more blood present), or for other questions/ concerns.

who can perform JP drain removal?

A Provider orA staff member who has been trained in drain removal by completing BOTH:

• Learning Exchange Module on drain care and
• Observation checklist proving competency of skill.

If you have not been trained in drain removal, you may assist with drain removal by preparing supplies, placing dressing after removal, and helping with red-bag disposal of drain.

STEPS: Jackson Pratt (JP) Removal

• Gather sterile suture kit and gauze.
• Perform hand hygiene and don gloves, gown, mask, and eye protection or face shield.
• Verify the correct patient using two identifiers.
• Explain the procedure and ensure that the patient agrees to treatment.
• Remove sutures
• Release suction by opening the bulb stopper.
• Instruct the patient to take a deep breath while stabilizing the surrounding skin with a gauze pad.
• Swiftly and evenly withdraw the drain. Do not force drain removal.
• If resistance is felt, stop and notify the practitioner.
• Inspect the drain after removal to ensure that it is fully intact
• No fragments were retained.

• Apply slight pressure to site with gauze for 3-5 minutes.
• For minimal drainage:
• Leave gauze in place, add additional layers as needed, and secure with a transparent dressing.
• For moderate drainage:
• Apply an absorptive, moisture-retentive dressing, such as a foam dressing.
• Label the dressing per the organization’s practice.

• Measure and record the amount of drainage in the collection device before discarding.
• Dispose of all soiled dressings, supplies, or single-use equipment in biohazard.
• Remove PPE and perform hand hygiene.
• Document the procedure in the patient’s record.

• RN/provider to assess, treat, and reassess pain.
