My Health Record contains health information like immunisations, pathology and diagnostic imaging reports, prescription information, hospital discharge summaries and other things. You can access My Health Record from your phone or computer. The EMR contains health information about patients during their hospital stay, and it can be accessed by the health care team caring for patients in hospitals.

Ask Questions I can prepare a list of questions before meetings or medical team rounds to better understand treatment and care decisions. Understand the EMR I can gain knowledge about how the EMR works and what information it contains by seeking explanations from health care workers. Request Clear Communication I can ask health care workers to give me clear updates about my care and treatment, especially during medical team rounds. Stay Informed after Leaving Hospital When plans are being made for going to hospital, I can ask health care workers about any changes to the treatment plan.
