Submit Documentation

In order to receive accommodations for courses, classrooms, or testing you must be registered with the Disability Services Office and approved by the Director of Disability Services.

Submit the appropriate documentation.

Submit a Letter of Accommodation Request

Complete and submit this form as soon as possible in the academic term (preferably during the first two weeks of the semester).

What's next?

What happens after I submit the form?

Letters of Accommodation must be requested at the beginning of every semester. Once the Coordinator will prepare the letters and contact students when they are ready for pick-up. It is the students responsibility to deliver the accommodation letters and meeting with their professors.
