Details of the Incident

At approximately 8:00 AM, a contractor was working on replacing a valve in the ammonia system. The contractor had been informed that all lockout/tagout (LOTO) procedures had been completed. However, the contractor did not verify the LOTO status before beginning the work. While disconnecting the ammonia line before the shutoff valve, a significant amount of ammonia was released, striking the contractor in the upper body. The contractor immediately exited the room, alerted security, and used the emergency shower. He is now experiencing confusion, dizziness, and difficulty maintaining balance. His skin is red, painful, and blistering. Additionally, he is complaining of shortness of breath and persistent coughing.

Immediate Hazards

Toxicity: Ammonia gas is highly toxic and can cause severe respiratory distress, burns, and eye irritation. Flammability: In high concentrations, ammonia can be flammable under certain conditions. Visibility: The release created a vapor cloud, reducing visibility in the area and complicating evacuation. Structural Integrity: Potential damage to nearby equipment due to exposure to the corrosive gas.
