This should be sent with EVERY resource packet (whether it is faxed, emailed, or mailed) to ensure the office gets into the habit of always sending a full package that includes a script, patient insurance, and clinicals.

These are great to send to offices with a pattern of patients not picking up our calls. Reminding the office staff to have their patients to save our number saves a tremendous amount of time going back and forth for the patient, the office, and us!
