Insureds must submit EFT requests through the County Office. IMPORTANT: Customer Service is NOT able to set a policy up on EFT over the phone unless your County Office uses DocuSign. EFT AUTHORIZATION & AGREEMENT FORM
- Insured must complete the EFT Authorization and Agreement Form.
- Send the ORIGINAL (top copy) of the form to the Home Office with the New Business Application or a Change Form.
- Is the insured setting up more than one policy on EFT? An original (top copy) of the EFT Authorization and Agreement form must be completed for each policy.
- Make sure the insured selects where the funds will transfer from: Either a Checking or Savings Account.
Establishing EFT Authority
When setting a policy up on EFT, make sure the insured understands:
- Initial, upfront payment is required.
- Initial processing may take up to 60 days.
- NCFB will prenote to ensure the accuracy of the banking information.
- An ACH prenote is a preliminary transaction verifying and validating bank account information before initiating an electronic fund transfer (EFT).
- The insured should continue to pay all non-EFT bills until they receive notification that EFT has been established for that policy.

Payment is Required for EFT Setup
When EFT is Requested at RENEWAL...
- If the renewal has already billed, collect the amount of the renewal bill.
- OR, if a payment plan is desired, collect the amount equal to one installment, plus the $3 service fee.
When EFT is Requested at NEW BUSINESS...
- For any payment plan, EXCEPT MONTHLY, you'll collect ONE INSTALLMENT plus, the $3 service charge.
- If the policy will be on a MONTHLY payment plan, we must collect TWO INSTALLMENTS up front, plus $6 for the service fee to cover both installments.
- We require two installments upfront for MONTHLY policies because it could take up to 60 days for EFT to be established. This is the ONLY time you would collect two installments.
