The first page has personal info
The second page begins with a place to make notes for the future and includes a checklist for the items/areas that must be addressed by law.
The second page includes the student's profile - this is a desciption - "present level of fuctioning" comes later. Click on the link to view requirements for a Reading Improvement and Monitoring Plan.
Students who would lose a significant amount of skills during the summer may receive extended school year (summer) services.

The rest of page one of the IEP includes other information that can be completed before the meeting.
If the student is more than 14 years of age, the transition plan must be written, with the goal of age-appropriate competitive employment.
As well as competitive employment, the district must address independent living preparation if appropriate, and how goals will be measured.

Any "specially designed services " (such as a 1:1 aide), related services (OT, PT, Speech, etc.) accommodations (scribe, extended time)
Transportation is often assumed - it needs to be noted on the IEP
An often ignored part of the IEP is participation in the school community.

Everyone's signature only means attendance, not agreement with the IEP. Parents always have due process. The student is a required member of the team once he or she reaches age 18 unless there is a transfer of guardianship.

The state booklet describing parents' rights must be offered at every meetiing.All rights are transfered to the student who reaches 18 unless there is a guardian appointed.
If students are blind or have visual impairments this checklist must be included.
