After a return to use

Team coordination

Throughout all phases:

• Maintain regular communication with counselor.

Should there be return to use

• remind the team appropriate response is not punitive
• call for reassessment of the treatment plan.

As patient enters programming:

Click the plus sign to review key components of coordinating care for each phase of the program from the medical provider's perspective.

• Add therapeutic adjustments and motivational enhancement approaches, as needed

• Perform functional analysis of the return to use.

Team Communications

Coordinate aspects of patient's program at entry:

• taking medication
• attending psychosocial counseling
• drug testing
• schedule appointments with staff
• attending court.
