CREATE INTERVENTION PLANS IN SIMS
Intervention plans are made from
Ad Hoc groups created within SIMS. This process can be expedited by selecting all students within an intervention group and creating one group intervention plan which applies to all these students.

CREATE ILP-Ds
Screening results may indicate a student has characteristics of dyslexia (COD) and require dyslexia-specific intervention. The MNPS ILP-D Decision-Making Crosswalk provides guidance for determining students who demonstrate COD and require an Individualized Learning Plan for Characteristics of Dyslexia (ILP-D). ILP-Ds are
created in TN Pulse. This document provides guidance on creating an ILP-D and parent
notification. This separate document provides guidance on finalizing the ILP-D.
SEND FAMILIES STUDENT INFORMATION PLAN & LETTER OF PROGRESS
After 5-week data team meetings are held, and intervention plans are reviewed, clicking on the “Create Document” button within student intervention plans will generate the Student Intervention Plan & Progress Letter as a PDF. Student Intervention Plan & Progress Letters are individual to each student, and can be translated into the student’s home language, as listed on Infinite Campus.
SEND FAMILIES NOTIFICATION OF INTERVENTION
After intervention plans are created in SIMS, the “Create Document” button generates a notification letter as a PDF. Notification letters are individual to each student, and can be translated into the student’s home language, as listed on Infinite Campus.
RECURRING 5-WEEK DATA TEAM MEETINGS
Every 5 weeks, data teams convene to review student progress. The Data Teams Meeting page within the MTSS SharePoint provides resources for
preparing for and conducting these meetings. Data Meeting Minutes and
Academic Intervention Monitoring guidance can support the team in reviewing student data.
INITIAL DATA TEAM MEETING
Multiple sources of data (including screening information) inform decisions around tiering students for intervention. Screening to Intervention Report recommendations (S2i)
and SIMS Early Warning System (EWS) alerts support decision-making around tiering students according to their general and specific areas of need.
UNIVERSAL SCREENING
Universal screenings occur three times during the school year for all students. Screening measures are specific to grade band and time of year. Screening includes a combination of FastBridge
assessments and measures that look for characteristics of dyslexia. Assessment binders contain student-facing materials for screeners. Teachers record student performance within
corresponding platforms. Assessment team resources contain further preparation guidance and instruction for conducting universal screening.
SET UP PROGRESS MONITORING IN FASTBRIDGE
Bi-weekly progress monitoring is aligned to the general and specific areas of need identified for each student during the initial team meeting. FastBridge Progress Monitoring Recommendations can help in determining which progress monitoring probes to include in student plans.
