Pluridisciplinarité

" pluri " (plusieurs), désignant l'implication de différentes disciplines travaillant ensemble sans intégrer leurs savoirs.

Interdisciplinarité

Du latin " inter " (entre) et " disciplina " (apprentissage.

Historique

L'idée de colaboration entre disciplines remonte à la Renaissance. Au XX ème siècle, l'interdisciplinarité est devenue un concept valorisé en sciences, notamment pour résoudre des problèmes complexes.

Interdisciplinarité

--> Collaboration active entre différentes disciplines pour échanger, intégrer et enrichir les savoirs.

Définitions

Pluridisciplinarité

--> L'objectif est de prendre en charge le patient d manière globale et holistique.

--> Plusieurs disciplines qui travaillent en parallèle sur un même sujet, chacune avec sa propre approche.

Interdisciplinarité

Pratique professionnelle

--> Indispensable pour la prise encharge globale et holistique du patient, où les soins doivent être coordonnés entre les disciplines.

--> Collaboration active entre différentes disciplines pour échanger, intégrer et enrichir les savoirs.

Pluridisciplinarité

--> Utilisée lorsque chaque professionel apporte sa perspective spécialisée sans nécessité une intégration étroite avec les autres disciplines.

Ex : réunion de synthèse pour construire un plan de soins incluant différents professionnels qui partagent leurs observations pour élaborer une stratégie commune.

Ex : le médecin, l'infirmier, le kiné travaillent chacun sur leur partie de prise en charge.

La prise en charge du patient en milieu de soins de suite et de réadaptation après un AVC implique la participation de multiples acteurs : médecin, IDE, AS, kiné, ergothérapeuthe, orthophoniste, psychologue, psychiatre, diététicien...

Situation de stage

Chaque acteur contribue à une prise en charge complète, centrée sur les besoins du patient, afin de favoriser la réadaption et le retour à l'autonomie. Chacun joue un rôle spécifique en collaborant pour assurer une prise en charge adaptée, complète et centrée sur les besoins spécifiques du patient.

• Médecin : Diagnostique et coordonne le traitement médical.
• Infirmière : Assure les soins quotidiens et surveille l'état de santé.
• Aide-soignant : Aide aux soins de confort (toilette, alimentation).
• Kinésithérapeute : Travaille sur la mobilité et la rééducation physique.
• Ergothérapeute : Aide à retrouver l'autonomie dans les activités quotidiennes.
• Orthophoniste : Travaille sur la communication et la déglutition.
• Psychologue/Psychiatre : Apporte un soutien psychologique.
• Diététicienne : adapte le régime alimentaire en fonction des besoins.
