A 3-year child who was swimming with her family, when she wandered into the deep end and submerged under water. The parents noticed the child was below the surface and brought her to the surface. The parents are unsure exactly how long the submersion was. The child was unconscious but breathing spontaneously. She was noted to be coughing up a white frothy substance periodically. Mom brought the unconscious child to the pool deck, while dad called 911. It took < 15 minutes for EMS to arrive. Upon EMS arrival, the child had a pulse of 100 bpm and was spontaneously breathing. Despite 100% FiO2, her SPO2 remained 90%. She was transported to ED.
