¿Que es el TCCC?

Tactical Combat Casualty Care, que en su traduccion significa Cuidado Táctico de Heridos en Combate. Se trata de un conjunto de pautas para brindar soporte vital a personas heridas en combate, con el objetivo de reducir las muertes evitables.

EMPLEO DE LA MEDICINA TACTICA

La Medicina Tactica fue creada para aumentar las posibilidades de supervivencia de los combatientes heridos durante operaciones bélicas. Las intervenciones del TCCC son efectuadas durante una serie de etapas denominadas fases de atención. Son 3 las fases en las que se dividen las directrices, Care Under Fire (Cuidado Bajo Fuego), Tactical Field Care (Atencion Tactica en el Campo) y Tactical Evacuation (Evacuacion Tactica).

De las muertes en combate se producen antes de recibir atención médica, de las cuales el 24.3% podrían sobrevivir con una adecuada atención sanitaria.

Cuidado bajo el fuego

Es la primera fase de atención en el campo de batalla durante la cual la víctima y el sanitario están bajo fuego hostil. El objetivo principal consiste en detener o disminuir el fuego enemigo en la mayor brevedad posible. El objetivo principal es tratar los focos de hemorragias.

Actuaciones asistenciales.

• Uso de Torniquetes o agentes Hemostaticos.
• Revertir la obstruccion de vias respiratorias.

ATENCIÓN TÁCTICA EN EL CAMPO

Corresponde a los cuidados ofrecidos por el personal sanitario a los combatientes heridos cuando la unidad ya no se encuentra en una zona caliente o de peligro por fuego enemigo

Todas las armas deben recogerse de cualquier víctima con estado mental alterado para evitar su empleo involuntario o inadecuado.

Head trauma/ hypothermia

Massive bleeding

Manejo de traumatismos craneoencefálicos y prevención de la hipotermia con medios activos.

Control de hemorragias mediante: Torniquetes, agentes hemostáticos, vendajes compresivos y torniquetes de unión de miembros.

Es empleado en la atención rápida en un terreno donde los recursos y el tiempo son limitados, evalúa y trata sistemáticamente a los pacientes con el fin de posponer la muerte de las víctimas el tiempo preciso para ser atendidos en el centro hospitalario. En primer lugar, se pretende controlar las hemorragias masivas dado que en tan solo 3 minutos podría fallecer el combatiente.

Circulation

Airway

Manejo de problemas circulatorios, como el shock hipovolémico mediante: Ácido Tranexámico, sangre fresca total o hemoderivados calientes. Si no se dispone de sangre o hemoderivados, se procede a la reanimación hipotensiva con coloides o cristaloides calientes.

Respiration

Manejo de problemas respiratorios por problemas torácicos, como: Colocación de parches torácicos en heridas penetrantes en tórax. Descompresión de neumotórax a tensión con aguja/catéter.

Manejo de la vía aérea con: Cánulas nasofaríngeas, supraglóticos, cricotiroidotomía quirúrgica, intubación orotraqueal
