This simulation should take you just over two hours, and is divided into the following sections:

  • A preparation task, which should take you up to 15 minutes
  • A 30 minute research task
  • A 30 minute analysis task
  • A 30 minute create task
  • A 10 minute document and presentation activity
  • And finally, a reflection task, which should take you around 10 minutes.

This simulation will provide practical experience in conducting a thorough nursing assessment in a pre-hospital environment, including gathering patient history, performing physical examinations, interpreting vital signs, and documenting findings.As a reminder, below are the steps we're going to take in order to complete the simulation. There will be timers on each step, but this is simply there for guidance and there is no pressure to finish before or after the time is up!

PREPARAtion task

This part will set the context and prepare you mentally for the nursing assessment process.

  • Task: Review the basic principles of nursing assessment within the context of emergency care. Focus on the importance of:
  • the ABCs (Airway, Breathing, Circulation)
  • history taking
  • the head-to-toe assessment
Outcome: Clear understanding of the key components of a nursing assessment and its relevance in the paramedic role.

This part will help you gather essential information about the patient before initiating a full assessment.

  • You are presented with a patient scenario (a 45-year-old male involved in a car accident).
  • Use the first 10 minutes to formulate a brief patient history from the scene:
- bystanders- if the patient is conscious- medical tags or identification cards
  • Spend the next 10 minutes researching common injuries and conditions associated with the scenario using available reference materials.
Outcome: A collected set of potential conditions or injuries to consider during the assessment.

Collecting data and information:This part will help you to interpret and prioritise the gathered information to guide the physical assessment.Based on the information gathered in the research task, identify the most likely conditions or injuries. Use a differential diagnosis approach to prioritise what to assess first during the physical examination.Outcome: A clear and prioritised plan for conducting the physical examination.

Perform a comprehensive nursing assessment and document findings.Task:

  • Describe in one paragraph what ‘a DORA’ means, and why it is vital to use when attending an accident.
  • Describe in one paragraph how you would conduct an Airway, Breathing, Circulation assessment (10 minutes): Assess and stabilise the ABCs.
  • Describe in one paragraph how you would conduct a head-to-toe physical assessment on the patient following a prioritised plan.
  • Describe in one paragraph how you would conduct a head-to-Toe Assessment (15 minutes): Systematically check for injuries or abnormalities.
  • Describe in one paragraph how you would Reassess and Record (5 minutes): Review and document the findings.
Outcome: A complete nursing assessment documented with all relevant data captured.

Compile the assessment findings into a concise and accurate report.Using your notes, create a structured report that includes:

  • The patient’s history
  • Physical exam findings
  • Vital signs
  • Any interventions performed
Present this report in a Patient Report Form using this structure:
  • PC - presenting complaints
  • HPC - history of presented complaints
  • OA - on arrival (initial findings)
  • OE - on examination (A-E assessment)
  • PMH - past medical history
  • MEDS - what are they taking?
  • ALLERGIES
Outcome: A detailed and well-organised nursing assessment report ready for review.

Reflect on the Process

Reflect on the assessment process, the decisions made, and the overall experience.

  • Reflect on the assessment, considering what went well and what could be improved.
  • Focus on areas like communication, decision-making under pressure, and thoroughness of the assessment.
  • Write a brief reflection (100-150 words) discussing how this experience can inform future nursing assessments.
Outcome: A reflective piece that provides insights into your assessment skills and identifies areas for growth.
