Preparation task (10 minutes):

As part of this simulation, you’ll be working as a Nurse in your local NHS Trust Foundation Hospital. You will create a comprehensive care plan for a patient, integrating assessment skills, relevant patient information, symptom management, precautionary measures, therapeutic goals, and continuity of care. Before you get started, you need to understand what a basic patient profile looks like. Objective: Understand what a basic profile for a hypothetical patient looks like. Step one: Research basic patient profiles. After conducting your research, consider what warning signs you would expect from a patient like the one below. Have a look at this basic profile for a hypothetical patient:

  • Mr. Joe Biggs
  • 65-year-old male
  • Diagnosed with:
    • COPD
    • Hypertension
    • Type 2 diabetes

Research task (30 minutes):

In this next part of the simulation, you’ll conduct research into the hypothetical patient’s diagnosed diseases, and you’ll also be encouraged to think about disease management guidelines, common complications, and your role as a Nurse in managing patients such as Mr. Joe Biggs. Objective: Learn more about different diseases, including their management guidelines and complications, and understand the role of a Nurse in managing patients with diseases such as the ones described.

Step one: Research the patient’s diagnosed diseases, including their management guidelines and common complications, and reflect on the role of a Nurse in managing patients with these diseases. Research the diagnosed diseases, then for each of the patient’s diseases, conduct further research into:
  • Standard management guidelines for the disease
  • Common complications of the diseases
  • The role of a Nurse in managing such patients

Analysis task (30 minutes):

This part of the simulation requires you to take part in an analysis task, where you’ll consider a patient’s needs. Objective: Understand what different psychosocial and healthcare needs a patient with diagnosed diseases might have.

Step one: Do some research into what psychosocial and healthcare needs a patient with these diagnosed diseases might face.

Step two: Consider what challenges this patient might face.

Based on the disease profile and the patient's situation, analyse the patient's psychosocial and healthcare needs.

Create task (20 minutes):

This next part of the simulation requires you to create a comprehensive care plan for the patient. You’ll need to consider a number of different factors to ensure the patient’s psychosocial and healthcare needs are met. Objective: Understand what factors must be considered when formulating a comprehensive care plan for a patient.

The next part of the simulation involves you formatting your care plan from the previous task into a structured document.

Reflection task (10 minutes):

In this final part of the simulation, you’ll complete a self-evaluation task which encourages you to reflect on the process of creating the plan. Objective: Undergo a self-evaluation where you reflect on what was straightforward about the task, what you found challenging, and what you have learnt about the role of a Nurse in providing care for patients with diagnosed diseases.

Step one: Review your work and think about what you found challenging about completing the task, what was straightforward, and what you have learnt about a Nurse’s role in providing quality care for patients. Reflect on the process of creating the care plan.

  • Application: What aspects were straightforward?

Expected Outcome

By the completion of this activity, the participant should gain practical insight into care plan formulation for a patient suffering from diagnosed diseases. The ability to understand patient needs, construct an appropriate care plan, document it efficiently, and reflect on one's performance are integral skills for modern-day nursing professionals.

Create task

Create a comprehensive care plan for the patient. The plan should include:
  • Physical Care: What daily care and monitoring does the patient require? Consider mobility, hygiene, respiratory support, and vital signs.
  • Pharmacological Symptom Management: What medications might be prescribed, and what is the nurse’s role in administering and monitoring them?
  • Non-Pharmacological Management: What lifestyle changes or supportive therapies (e.g. breathing exercises, physiotherapy) could help manage the patient’s symptoms?
  • Nutrition and Hydration: What dietary needs or restrictions should be considered, and how can the patient be supported to meet them?
  • Involvement of the Multidisciplinary Team: Which healthcare professionals should be involved, and what roles do they play in the patient’s care?
  • Psychosocial Support: What emotional, mental health, or social challenges might the patient face, and how can they be addressed?
  • Education about Condition: What does the patient need to understand about their diagnoses, medications, and self-management?
  • Discharge Planning: What steps need to be taken to prepare the patient for safe discharge, including follow-up care or community support?
