Lily is 79 years old. Her husband died a year ago and she lives alone. She has lost approximately 15 pounds since he died. Her family is beginning to notice that her clothes are rather loose, and they are concerned about her health. She has told them that sometimes she forgets to eat, and she finds it hard to decide what to cook for just herself. She also complains that foods taste bitter or bland. She still drives and shops for herself. She has three children who live fairly close, but they are all busy with their jobs and families, so she sees them each approximately once a month. She is actively involved in her church and attends a quilting group once a week.
Assessment as follows:
VS: 97.0 F, HR 95, RR 16, BP 90/50, pulse ox 96% on RA
HR irregular, pulses 2+ radius and pedal; breath sounds clear bilat., respirations nonlabored; Abdomen is soft, flat, with active BS, cracked corners noted to mouth; skin is dry, flaky, pale with bruising, weak but equal strength to extremities.
