Fox Forms are submitted to the VA through the Data Collection Tool which is found at https://onboarding.ssg.vaec.va.gov/home.

Complete screening assessments. If they score "No risk" on the Columbia, they are ineligible. Submit an Action Form stating they are ineligible, any referrals you made, and you are done! If they score "Low/Moderate" they are eligible for services. Submit an Action Form for any referrals you made and proceed to Enrollment Assessments. If they score "High", you must submit an Action Form for what referrals you made and go to Enrollment Assessments.
Start by completing intake.
These assessments officially enroll the Veteran in Fox. After completing each, the Action Forms communicate referrals and their current standing in the program. To exit the program, complete an Action Form and select the appropriate result.
The last step for those who complete the program is the Satisfaction Survey.
