Inpatient Social Services will receive an automated consult referral to provide additional resources to members at risk patients based on documented screening responses.

If the patient is able to conduct the screening:

• Engage the patient by explaining what social health is and how it is an important part of an individuals total health.

• Express that Kaiser Permanente may be able to provide support.

• Ensure confientiality in all answers and conversation.

• Begin SDOH Screening.
