Provide Appropriate Identification
Donors must present a valid form of photo identification at the start of the collection process.
Acceptable ID's
- Driver's license
- Employee badge (issued by the employer)
- Federal, State, or local government issued ID
- School ID
Verify Identity if no ID
If the donor cannot provide identification, contact the Point of Contact (POC) listed on the protocol to verify the identity.
Do Not proceed without ID
The collection process should not continue untill positive Identification is confirmed.
Report Issues
If the collection cannot be completed, contact the Designated Employer Representative (DER) immediately.

- Can not be a co-worker or another safety-sensitive employee
- Donor must provide two items of ID with their signature.
- Accepted forms depend on the specific DOT administration (FMCSA, FAA, FRA, FTA, PHMSA) regulations and guidance documents detail acceptable IDs.
- Compare the donor's signature.
- If signatures match, proceed with the collection.
- If signatures do not match, note "signature verification is unconfirmed" in remarks.
