Call Out Procedure

Call the Patient Transport Dispatcher between 5:00am-11:30pm so they can record the call out. There is no one to accept a call outside of those hours. If the dispatcher is not notified of a call out before the second hour of the associate's scheduled shift, then it will be counted as a No-Call, No-Show.

PTO (Paid Time Off) Request Process

Blank PTO forms can be found in the Patient Transport office and in the Switchboard office next to the time clock. Completely fill out the top portion of the form with the dates and times that you will need off. Once completed, turn into manager's box and an approved/declined copy will be returned to you for your records.

Patient Transport is open dailyfrom 5:00am - 11:30pm Switchboard is open 24 hours / 7 days a week
