Level 2:Access to the HMIS System not including access to the Coordinated Entry Agency
This means your agency already has an active program in the HMIS system. This is the only access JoCo can requestTo both conduct assessments and attend CES Case Conferencing meetings, providers must
- Able to enter data into the HMIS system for your primary agency.
- Not able to conduct or add CES Assessments, enroll households in CES, add to CQs, etc., no access to case conferencing unless previously stated requirements have been met.
Level 3: Access to Coordinated Entry Agency within HMIS
This means your agency can assess individuals using the paper assessment and join the CES Case Conferencing meetings. Balance of State ONLY!

Level 4: Access to the HMIS System + Access to Coordinated Entry Agency within HMIS
This means your agency has an active program in the HMIS System, and you would like access to CES in Clarity. These individuals are under their primary agency in Clarity and have been granted additional access to the Coordinated Entry agency in Clarity. Providers are able to
- Enter data into the HMIS system for their primary agency,
- conduct and input assessments, attend case conferencing,
- request CE referrals, accept/deny CE referrals.

Fleeing/Attempting to Flee DV
Individual or family who lacks a fixed, regular, and adequate nighttime residence, meaning:
- Has a primary nighttime residence that is a public or private place not meant for human habitation;
- Is living in a publicly or privately operated shelter designated to provide temporary living arrangements (including congregate shelters, transitional housing, and hotels and motels paid for by charitable organizations or by federal, state and local government programs); or
- Is exiting an institution where (s)he has resided for 90 days or less and who resided in an emergency shelter or place not meant for human habitation immediately before entering that institution
Any individual or family who:
- Is fleeing, or is attempting to flee, domestic violence;
- Has no other residence; and
- Lacks the resources or support networks to obtain other permanent housing
Unaccompanied youth under 25 years of age, or families with Category 3 children and youth, who do not otherwise qualify as homeless under this definition, but who:
- Are defined as homeless under the other listed federal statutes;
- Have not had a lease, ownership interest, or occupancy agreement in permanent housing during the 60 days prior to the homeless assistance application;
- Have experienced persistent instability as measured by two moves or more during in the preceding 60 days; and
- Can be expected to continue in such status for an extended period of time due to special needs or barriers
Assessment
Core Components
Assessment is the process of gathering information about a person presenting to the crisis response system.
Assessment includes documenting information about the barriers the person faces to being rapidly housed and any characteristics that might make him or her more vulnerable while homeless.
The KSHC uses a custom assessment named the CES assessment. Paper copies of the assessments can be found in the on the CES page of the KSHC site.

Core Components
Access refers to how people experiencing a housing crisis learn that coordinated entry exists and access crisis response services.
The first contact that most people experiencing a housing crisis will have with the crisis response system is through a coordinated entry access point. If you are interested in becoming an access point, send the CES team a Freshdesk ticket. Links

Rapid Exit/Rapid Resolution
Rapid Resolution is a housing intervention designed to prevent immediate entry into homelessness or to quickly resolve a household’s homelessness once they enter shelter, transitional housing, or an unsheltered situation.
After a household becomes homeless and enters the shelter system, Rapid Exit strategies help them find stable long-term housing.
Permanent Supportive Housing
PSH
A project that offers permanent housing and supportive services to assist homeless persons with a disability (individuals with disabilities or families in which one adult or child has a disability) to live independently.
Rapid Exit/Rapid Resolution
Rapid Resolution is a housing intervention designed to prevent immediate entry into homelessness or to quickly resolve a household’s homelessness once they enter shelter, transitional housing, or an unsheltered situation.
After a household becomes homeless and enters the shelter system, Rapid Exit strategies help them find stable long-term housing.
Referral
Core Elements
The group of persons with the highest priority must be offered housing and supportive services projects first. CES Participating projects must receive at least 95% of their referrals through CES. A referral must be requested through the CES team for all CE Participating housing programs; this does not include;
- Homeless Prevention services
- Veteran Servies
- Victim Service Providers
- ES/SO enrollment
Transitional Housing
TH
A project that provides temporary lodging and is designed to facilitate the movement of homeless individuals and families into permanent housing within a specified period of time, but no longer than 24 months. Requirements and limitations may vary by program and will be specified by the funder.
Rapid Re-Housing
RRH
A permanent housing project that provides housing relocation, stabilization services, and short- and/or medium-term rental assistance as necessary to help a homeless individual or family move as quickly as possible into permanent housing and achieve stability in that housing.
Emergency Shelter
ES
A project that offers temporary shelter (lodging) for the homeless in general or for specific populations of the homeless. Requirements and limitations may vary by program and will be specified by the funder.
Prioritization
Core Components
The person’s level of vulnerability or need is determined by analyzing the information obtained from the assessment against the CoC’s prioritization standards.
It is the person’s prioritization status (and other information from the assessment) that determines where the person will be referred in the next coordinated entry step. The CES team ensures all outgoing referrals are prioritized so those with the highest needs are offered services first
Street Outreach
A project that offers services necessary to reach out to unsheltered homeless people, connect them with emergency shelter, housing, or critical services, and provide urgent, non-facility-based care to unsheltered homeless people who are unwilling or unable to access emergency shelter, housing, or an appropriate health facility.
