Why the updates?

We heard your concerns!

  • The current template is hard to navigate on the phone.
  • The current template takes too long to complete in the last 15-minutes of session.
  • There are errors or delays when selecting the targets ran.
We collected data and developed a solution for you!The updates will make the session note process faster, easier, and more concise, while still meeting medical requirements.

1st change: Session Narrative

There will only be one session narrative to describe at least three targets ran during session, behaviors that occured, intervention strategies used, and client responses. Click the + to learn more

Session Narrative

The session narrative is a required section. Tips & Recommendations:

  • Record personal notes using the notes app or on paper (must not record PHI) throughout session of the important events and details.
  • Copy and paste your notes to the session narrative section.
    • Edit the pasted note to ensure correct grammar and spelling.
  • Write events and details using third-person language.
  • Stay netrual - do not include personal opinions or assumptions.

2nd change: Session Summary Data

Instead of selecting 3-5 targets from a list, the new Select Session Summary button will automatically add all the targets graphed during the session. Click the + to learn more

Session Summary Data

The session summary data is a required section. Tips & Recommendations:

  • Confirm you have graphed all and ended your session before creating your note.
  • Confirm the date you are selecting is the date of the session you are converting.
    • Selecting a date different than the session date is an instance of fraud.
  • Select the correct session name if your client is assigned to more than one data sheet.
    • Selecting data that you did not collect during session is an instance of fraud.

3rd change: ABA Techniques Used

You are now required to mark all that apply under the ABA Techniques Used and Prompt Procedure Used sections. This was previously optional.

An awesome session narrative example

Can you point out why this is a well written session narrative?

During session, the client participated in several target activities including manding, following 2-step instructions, and tolerating denied access. Tantrum Behavior: At the beginning of the session, the client engaged in tantrum behavior when access to a preferred toy was denied. The tantrum included crying and dropping to the floor. The BS used visual aids to remind client of coping strategies to manage the behavior. After 5 minutes, the client transitioned to a preferred sensory item and the session continued. Manding: The client was prompted to use manding to request snack items. The client successfully used manding to request a preferred snack 4 out of 5 opportunities with 80% independence. The BS used verbal and gestural prompts to support the client in identifying and verbalizing the request. Verbal praise and a high-five were provided each time the client used manding. Following 2-Step Instructions: The client worked on following 2-step instructions, such as "get the ball and put it in the basket." The client followed the 2-step instructions independently 3 out of 5 times. For each successful attempt, the client received verbal praise and a sticker.

During the session, the client engaged in social skills training, focusing on initiating conversations, maintaining eye contact, and responding to social cues. The client practiced starting conversations with peers during activities with siblings. The BS used modeling and verbal prompts. The client initiated conversations 4 out of 5 times with 80% independence. Successful attempts were rewarded with verbal praise and points on the token board. The client worked on maintaining eye contact during interactions. The BS used gestrual and visual cues to support the client. The client maintained eye contact for at least 5 seconds 6 out of 8 times. Verbal praise and tokens were given for successful attempts. The client practiced responding to social cues, such as recognizing jokes and identifying when someone is upset. The BS used video modeling and discussions to teach these skills. The client responded appropriately 3 out of 5 times with 60% independence. Correct responses were rewarded with verbal praise and preferred sensory breaks.

Session Narrative

  • Details in the narrative must match the data graphed and collected during session
  • State the names of at least three targets ran during session and,
    • The total amount of trails ran and how many trials were successful
    • Intervention used, including prompts.
    • Client responses to interventions.
    • Reinforcement provided.
  • State any challenging behaviors that occurred and,
    • Proactive and reactive strategies implemented.
  • Include other client treatment program information that should be mentioned.
  • Statements must use measurable and objective language only. No assumptions, subjective statements, or opinions.
