Introduction
This module is designed to give guidance to clinicians in the “Peacocks Way” of prescribing, measuring, and supplying Orthotic Footwear. This module is also of use to the manufacturing team to ensure we all have a common understanding and expectation of the process of providing Orthotic Footwear.
This build on previous versions of the document, thank you to the previous contributors for their work.
Footwear is a special case amongst the range of devices that we provide. Most devices are by their design a medical device and only this. An AFO or wrist brace can be made as cosmetic as possible but will always look like an AFO or wrist brace.
With footwear the device is replacing something that would normally be worn. There is often an expectation that the orthotic footwear will look like shoes or boots, and not like a medical device.
Managing this expectation is part of the skill of the orthotist, however the prescription and manufacture can also work to meet expectations. Sometimes it will be necessary to compromise function to achieve this.

The Prescription process for any Orthotic Footwear, as for any Orthosis is as follows:
Pathology & Prescription
The pathology of the patient will tend to certain prescriptions. The following contains some general principles and guidance, but not every presentation will be typical of its pathology – every patient is unique.

Diabetes
Footwear provided for this condition is usually provided for protection and to prevent the re occurrence of previous ulcers. Therefore the construction is usually of softee leather with a manmade foam/leather lining and manufactured to have no internal seams. This is to minimise the areas likely to cause abrasion on the foot.
Care should be taken however that softee leather combined with soft lining is not too flexible, and either does not maintain its shape, or give sufficient control of the movement of the foot. For larger diabetic patients, firmer footwear should be considered.
Patients with Diabetes develop neuropathy where the distal nerves in extremities especially the feet, lose their ability for pain sensing as the myelin sheaths become degraded due to uncontrolled sugar level. This loss of innervation leads to reduced pain sensation, so the foot is more at risk as the patient is unaware that trauma developing. The shoes are normally provided with shock absorbent insoles to further reduce friction and prevent ulceration. Depending on the nature of the foot, rockers are provided to further off load previous ulcer sites and to reduce friction.
Up to 50% of diabetic peripheral neuropathies may be asymptomatic. If not recognized and if preventive foot care is not implemented, patients are at risk for injuries to their insensate feet.

Inflammatory Arthritis
The nature of this condition is that due to the progression of the disease, the foot can change shape as the joints swell and subside. Therefore, shoes are normally well-fitting footwear with lightweight sole units to absorb shock and provide lightness as patients often complain that footwear is heavy. Again, shock absorbent insoles are provided to further increase the shock attenuation and give the perception that the shoes are lightweight. Often, the insoles are used to provide some biomechanical correction to off load problematic areas. However, the very nature of the disease can lead to increased osteoarthritis which can prevent correction.

Measuring
The process in correctly measuring the patient's foot is the most essential part in the process in manufactuing footwear to fit appropriately. The following sections include:• Principles behind measuring

Principles behind measuring
When measuring for footwear, be it bespoke, stock or modular, the process is more than just accurately writing down what size the foot in front of you is.
The measuring process is the way in which the orthotist communicates to the technicians, what the footwear will be in terms of size, shape, style, and function, to meet the prescription requirements discussed previously.
For example, the Orthotist will know if the patient favours cosmesis and would prefer a shoe with a narrower toe box allowing a little pressure on the toes, if the high-risk diabetic requires space for the toes or if a patient tends to swell but is not swollen today. Allowances would be made for all these by the Orthotist.
