  • Integrated Social Service Model: Combines housing, healthcare, workforce development, legal, and social services.
  • Facilities & Security Offered: Below-market rate cost to operate at Pathways saving small-to-large nonprofits anywhere from $15,00 to $50,000 annually in overhead expenses.
  • Outcome-Oriented Approach: Focus on improving individual self-sufficiency, and collective impact: stability, health, and economic outcomes.

Client and Community Impact:

  • Our efforts provide clients with streamlined access to essential services at no cost, promoting self-sufficiency, employability, financial stability, and improved health outcomes. This approach improves housing conditions, reduces social isolation, and empowers clients with self-determination and well-being.
Community:
  • Collaborative initiatives yield socio-economic benefits, reducing homelessness and poverty and enhancing community health. Access to resources supports local businesses, creates jobs, and stimulates the economy, fostering a strong sense of community, resilience, and inclusivity.

Major Accomplishments / Milestones:

2018 Major Achievements:

  • Incorporation and IRS Recognition: Officially incorporated on January 22, Pathways achieved tax-exempt status by June, laying the foundation to enhance community health and support cultural diversity for underserved populations with social health disparities.
  • Strategic Facility Acquisition: In November, we acquired a historic building, demonstrating our dedication to preserving community heritage and repurposing it into a welcoming community center. Furthermore, we successfully advocated for the City of Rock Hill to install a bus stop directly, i. This initiative is crucial to removing systematic barriers, ensuring easy access to resources, and enhancing community connectivity.
2019 Major Achievements:
  • Leadership Establishment: Appointed our first Center Director, a strategic move towards operational excellence and improved care coordination.
  • Community Center Opening: By September, our doors opened, serving 200+ clients in the first few months alone, demonstrating our capacity for immediate impact and long-term value creation of Pathways.
2020 Major Achievements:
  • Collaborative Care Efforts: Initiated monthly Continuum of Care meetings, enhancing care coordination and collaboration.
  • Infrastructure Enhancements: Completion of The Haven Men's Shelter (27 beds), the establishment of The Manna House Food Pantry in March, alongside the Community Soup Kitchen (House of Agape) in September - all of which contribute to our multifaceted approach to healthcare and wellness—integrated care.
2021 Major Achievements:
  • Shelter Expansion: Bethel Shelters' acquisition of our space in October for Day Center operations, providing day respite, laundry services, showers, lockers, workforce development, and case management, exemplifying our strategy to enhance access to care.
2022 Major Achievements:
  • Community Beautification: A donor-funded courtyard renovation in June, including a flower garden and water feature, reflects our commitment to creating a welcoming, healing environment.
  • Agency Collaboration: By mid-year, Pathways’s eleven (11) agencies showcase our ability to foster collaborative partnerships and extend our mission’s reach.
2023 Major Achievements:
  • Safety and Shelter Expansion: The completion of a building-wide fire suppression system in March and the opening of The Cottage in April represent significant steps forward in providing safe, dignified shelter and comprehensive case management.
  • 5th Anniversary and Expansion: Our 5th Anniversary in November celebrated our past achievements and marked the beginning of an expansion to serve more homeless clients, demonstrating our ongoing commitment to community health needs.
  • Service Impact: Growing from 11 to 18 on-site service agencies resulted in over 85,000 service connections by year-end, underscoring our substantial impact on health disparities, social economics, and access to care.

In our journey to combat poverty and homelessness, we've learned that no one can do it alone. Collaboration is key, bringing together various agencies under the shared goal of guiding people toward self-sufficiency.
