Rebates are not available for members on the Consumer Plus Plan and Medicare.

Know Your Numbers & Get $50 First Steps Rebate First Steps ($50) Rebate (Only members having their biometric screening done at a doctor office visit are required to download and submit this completed form.) Know your numbers by completing an annual biometric screening (cholesterol, blood glucose, body composition, and blood pressure) at a Healthy Utah testing session or your annual preventive doctor visit, then agree to participate in two PEHP wellness activities from this webpage.

• To register for a Healthy Utah biometric screening, log in to your PEHP account and select "Register for Testing Session" under the Wellness menu. Or call us at 801-366-7300.
• Wellness activities must be completed within 6 months after your biometric testing date.
• Rebate checks will be mailed to the member within 2-4 weeks after biometric testing date or submission of the First Steps rebate form.

Take the Next Steps & Get $50 Rebate for Making Your Health a Priority Next Steps ($50) Rebate (All members must download and submit this completed form to receive the rebate). After completing the First Steps Rebate requirements, take the “next steps” by agreeing to participate in two additional PEHP wellness activities from this webpage.

• Selected activities must be completed within 12 months after your biometric testing date.
• Complete and submit the Next Steps ($50) Rebate form to PEHP Wellness via your PEHP account to receive the Next Steps Rebate.
• Rebate checks will be mailed to the member within 2-4 weeks after the Next Steps rebate form is received.
