Once aware of an incident, initial contact is important and should be done as soon as possible Goal: Initial contact shuld be made 2 days from the partner being notified Q: What if you cannot contact a client? Ask SureHands HQ as soon as possible, we are happy to help!

Completed Incident Intake Form to be sent directly to: quality@surehands.com Photos of the incident must be submitted of the area where the incident took place, including:- Transfer Area
- Where the injury occured
- Any and all equipment present, damaged or not

To be included in the Incident Form:
- Is the lift system damaged?
- Did they seek medical attention?
- Any issues with the lift or accessories noticed prior to event?
- Is equipment available for review and or return for inspection?
- If needed on a case by case basis

- Customer Acknowledgement Form
- Last Yearly Inspection or Service documents
