Identification Information Must Be

• Driver permit or license issued by a U.S. state or territory
• U.S. Government Identification Card
• Passport
• Alien Residency Card

• Valid
• Current

And one of the following

Identification Must Include the Applicants:

• Driver permit or license issued by a U.S. state or territory
• Identification card issued by any government entity

• Photo
• Date of Birth
• Signature
• Physical Residential Address
