Step 1

Open your web browser and go to forms.office.com. Sign in with your Microsoft account. If you don't have one, you'll need to create it.

Step 2

Create a New Form: Click on the "+ New Form" button to start a new survey. Give your form a title by clicking on "Untitled form" and typing in a name. Add a description to provide context to your respondents.

Add Questions: Click on the "Add New" button. Choose the question type (multiple choice, text, etc.). Enter your question and options (if applicable).

Customize Questions: Use the toolbar to format text, add images, or include media in your questions.

Add Sections (Optional): To organize your survey into sections, click on the "Add new" button and select "Section."

Rearrange Questions: Click and drag questions to rearrange their order within a section.

Step 7

Required Questions: Toggle the switch to make certain questions required.

Style and Background: Click on the "Style" button to choose a theme and background color for your form.

Step 9

Settings: Click on the three dots in the upper-right corner to access settings. Configure options such as allowing multiple responses, accepting responses only from your organization, etc.

Preview Your Form: Click on the "Preview" button to see how your form will appear to respondents.

Share Your Form: Click on the "Collect Responses" button. Choose a method to share your form (copy link, send via email, embed, etc.).

Step 12

View Responses: Click on the "Responses" tab to view and analyze responses. You can also export responses to Excel for further analysis.

Step 13

Close and Reopen: In Settings, you can set start and end dates for responses. You can also manually turn this feature on and off.
